London Underground's safety chief has said sorry for wrongly claiming that government inspectors gave the tube a clean bill of health after a derailment.

Private emails read by the Guardian show that Mike Strzelecki, director of safety, was censured over assurances he issued two days after an accident at Hammersmith on the Piccadilly line.
