The government is to phase-in a new system in which NHS hospitals will received flat rate payments for treatment and operations.

The move is an attempt to rid the NHS of inefficiency.

It comes as the BMA chief Dr Ian Bogle warns that "we have a system driven not by the needs of patients but by spreadsheets".
