A DTI report into Robert Maxwell's business empire reveals a long series of oversights by leading city institutions - from auditors to bankers. The report has held back from recommending far reaching changes to rules and guidelines governing the city.

The report is to outline eight "modest" proposals, most of which are already in place.

The investigation also finds that the corrupt businessman first started plundering his companies' pension funds as far back as 1985 - six years before his death.
